Hole spacing when drilling 5/8 hole into stem wall

if I am attempting to drilling a 5/8 hole 4” embedment to epoxy a #4 rebar dowel into a 6” stem wall but continue to hit existing rebar before finally missing bar and getting required depth. What is the recommended distance between the holes, and if I am within the minimum at what point do I have to fill all failed holes with epoxy
