We are installing 3/8 Hilti KTBZ anchors 3 3/4" long with 2" imbedment for new handrail on a second floor at Longden Elementary school.
About 25% of the anchors are failing, the Concrete is about 50 years old and is light weight concrete.
I think that the failures that i am having is because of the light weight concrete using a pummis aggreget.
And because the concrete has fractures in it and we are hitting voids in the concrete.

Indeed concrete can be why you encounter failing anchors. Maybe we can help you to find another solution that could reduce the failure risk, through an adhesive anchor possibly if we can find one answering all your criteria.
