Epoxy anchor installation

We have this project and we are using 1" HAS rod, 12" embed, RE-500 Sd epoxy. It has been brought to my attention perhaps there is an issue with the installation. It seems some of the rods installed are slightly out of plumb, we can say this because of the projection of the rod over the concrete surface, there are some that are obviously vertical and some that are not. We don't know at this moment if the hole drill was out of plumb, and then the rod was inserted to be concentric to the hole, or the drilled hole was performed vertical and the rod was inserted skewed. We disregarded the possibility of a bent rod, since they are thick and about 18" long, so and bent rod would have been very obvious. Besides there are plenty of them on a box and they look straight. My question is about the case with vertical hole, inclined rod: since the drill bit diameter is just 1/8" bigger than the rod diameter, a skewed rod would indicate there is a non-uniform distribution of epoxy along the embedment (the rod would be in contact with the concrete at its top and bottom of the hole), the load capacities shown in charts are still applicable?
Also, from viewing videos of installation, the amount of epoxy over the concrete surface after the rod is installed, is it definitive more that what the photo shows, does this mean they applying less epoxy resin in the hole?

Pablo,

Per ACI 355.4 Section 4.5.1.3, drilled holes for anchors shall be within six degrees of perpendicular to the surface of the concrete member. If the hole is not perpendicular and the rod is not in contact with adhesive, this would be outside the scope of testing, and there is not a known reduction factor for this scenario.
